**General information on when the first period comes after a termination, how post-procedure bleeding differs from a period, what to do if the period is late, the chance of a new pregnancy and contraception.**

**Short answer:** After a termination, the first period usually comes within 4-6 weeks. Ovulation, however, can start before the first period, on average 2-3 weeks after the procedure; so it is possible to become pregnant again before the first period, and if pregnancy is not wanted, contraception should start straight away. The bleeding in the few days after the procedure is not a period. If no period has come within 6-8 weeks, a pregnancy test and an examination are needed.

In my practice as an obstetrician and gynaecologist in the Muratpasa area of Antalya, two of the questions patients ask most often after a termination (ending a pregnancy, or clearing the womb after a miscarriage) are when their cycle will return to normal and when pregnancy becomes possible again. For the procedure itself, see our [termination of pregnancy](https://www.muratalpay.com.tr/en/tedaviler/kurtaj/) page; for the recovery period, see our article on [recovery after a termination](https://www.muratalpay.com.tr/en/blog/kurtaj-sonrasi-iyilesme-sureci-nelere-dikkat-edilmeli-antalya/).

After a pregnancy has ended, it takes a certain amount of time for the body to recover and for hormones to return to their pre-pregnancy levels. To help you manage this period with confidence, I have summarised the most frequently asked questions below.

## When Does the First Period Come After a Termination?

After a termination, the pregnancy hormone (beta-hCG) is gradually cleared from the blood and the ovaries return to their normal working pattern.

- **Expected interval:** The first period usually comes **within 4 to 6 weeks** of the procedure. This can vary with how many weeks pregnant you were, your hormone levels and your own menstrual pattern.
- **Ovulation comes earlier:** Ovulation can restart before the first period, on average **2-3 weeks after** the procedure (sometimes as early as 8-10 days).

Abortion is the ending of a pregnancy in the womb and the emptying of the womb. In Türkiye it can be carried out on request until the 10th week of pregnancy is complete; the week is confirmed by ultrasound.

## How Many Days Does Bleeding Last After a Termination? How Is It Different From a Period?

The bleeding or spotting seen after the procedure usually lasts **3 to 7 days**; in some women it can taper off over up to 1-2 weeks. Red in the first days, the bleeding gradually turns brown and fades to spotting.

This bleeding is not a true period. It occurs while the lining of the womb (the endometrium) heals and the womb returns to its former size. You should count your cycle not from the day this bleeding started but from the first true period, which comes a few weeks later.

If bleeding is heavy enough to soak more than one large pad an hour for two hours, or if there are large clots, a temperature of 38°C or above, pain that does not settle, or foul-smelling discharge, see a doctor without waiting.

## Is It Normal if the First Period Is Light or Heavy?

Yes, most of the time it is normal. Rebuilding of the womb lining and settling of the hormone pattern can take a few cycles. During this time it is expected for the first period to be lighter than usual (spotting) or temporarily heavier and irregular. Mild cramps may also occur. Over the next few cycles periods usually return to their former pattern; if periods remain noticeably lighter, an examination is needed.

## What Should You Do if Your Period Is Late After a Termination?

If more than 6 weeks have passed since the procedure and no period has started, the following possibilities come to mind:

- **A new pregnancy:** Because ovulation returns early, you may have become pregnant again through unprotected sex.
- **Tissue remaining in the womb:** A small piece of pregnancy tissue left behind can delay the cycle from settling.
- **Adhesions inside the womb (Asherman syndrome):** Rarely, after damage to the womb lining or an infection, adhesions (scar tissue sticking the walls together) can form inside the womb; periods may not come at all or may be very light.
- **Slow clearance of the pregnancy hormone:** Less often, the hormone level may fall more slowly than expected.

**What should be done?** If no period has come within 6-8 weeks, a pregnancy test should be done and an obstetrician and gynaecologist should be seen. At the examination the inside of the womb is assessed by vaginal ultrasound, to check for remaining tissue or adhesions.

## When Should a Pregnancy Test Be Done After a Termination?

After a procedure in the first three months, the pregnancy hormone is usually cleared from the blood **within 3-5 weeks**; depending on the starting hormone level, this can take longer. For this reason, a home pregnancy test done early may be positive even when there is no new pregnancy.

- For a home urine pregnancy test, waiting **about 3 weeks (21 days)** after the procedure is advised.
- If the test is still positive after 3 weeks, you should go to a healthcare facility to distinguish between conditions such as remaining tissue, an ongoing pregnancy or an ectopic pregnancy (a pregnancy outside the womb).

## Can You Get Pregnant Straight Away After a Termination?

There is a common but mistaken belief that pregnancy is not possible for a long time after a termination. On the contrary, **fertility returns very quickly after a pregnancy has ended.** Because ovulation can start before the first period, it is possible to become pregnant again through unprotected sex even before the first period. If a new pregnancy is not planned in the near future, contraception should not be put off.

## When Should Contraception Start After a Termination?

To prevent an unwanted pregnancy, contraception should be planned with your doctor immediately after the procedure or in the first few days.

- **After procedures in the first three months:** A contraceptive implant under the skin or an intrauterine device (coil) can be placed **within the first 5 days** after the procedure; placed in this window, it provides protection without the need for an additional method.
- **After procedures at later stages:** The suitable method and when to start are planned with your doctor according to how the procedure was carried out.

## When Is the Right Time for Those Considering Another Pregnancy?

If you have had a pregnancy loss and want to have a baby again, your body is generally ready for a new pregnancy once you have had your first normal period. The right time is once healing of the womb lining has been confirmed by examination and ultrasound, and when you also feel psychologically ready.

## Frequently Asked Questions

**How many days after a termination does a period come?**

In most women the first period comes 28-42 days (4-6 weeks) after the procedure.

**Does bleeding after a termination count as a period?**

No. The bleeding in the few days after the procedure is due to the womb healing; the true first period comes a few weeks later.

**Can you get pregnant after a termination before your first period?**

Yes. Because ovulation can start before the first period, on average within 2-3 weeks, pregnancy is possible through unprotected sex.

**My period has not come for 2 months since the termination. What should I do?**

You need to do a pregnancy test and be examined. A new pregnancy, tissue remaining in the womb or, rarely, adhesions inside the womb are investigated.

Remember to stay in touch with your doctor so that the process can be followed closely and the family planning method most suitable for you can be chosen.

Wishing you good health.
